“With increasing elderly population and number of people living with chronic diseases, it has become very important to enlist patients in their own long term care. Roughly 10-15% of population at any time is in need of regular healthcare engagement. Geriatric population and people living with Chronic illnesses are increasing by 15-30% every year. There is a large unmet need for enablement of digital solutions in chronic care, reaching patients outside the hospital surroundings and at home,” says Neelesh.

Healtho5 has designed their platform to help engage patients by trained nurses using a machine learning enabled patient health record. Patients are provided targeted and personalized communication plus coaching to improve health outcome. The platform also uses clinical and patient reported data to segment patients into risk profiles, flagging at-risk patients. They now use ML to analyze their extensive data (TensorFlow /Amazon ML / Apache Mahout) to identify at-risk patients. Healtho5 started with engaging 100 patients a month in May 2017, and now engage more than 3000 unique patients every month in specialities of HIV and Neurology, growing 30 times in ten months.
